    <ns1:description language="de">Kaiserschnitte stellen besondere Anforderungen an die AnÃ¤sthesie, da sowohl die BedÃ¼rfnisse der HÃ¼ndin als auch die der Welpen beachtet werden mÃ¼ssen. Einerseits muss die Narkose fÃ¼r die HÃ¼ndin tief genug sein, andererseits soll es nicht zu einer Depression der Welpen kommen, damit diese nach der Entwicklung schnell anfangen zu atmen. Um die Depression der Welpen so gering wie mÃ¶glich zu halten, werden nur sehr kurz wirksame AnÃ¤sthetika eingesetzt und die Entwicklung der Welpen so schnell wie mÃ¶glich durchgefÃ¼hrt. Der wichtigste Einfluss auf das Ãberleben der Welpen ist jedoch eine mÃ¶glichst kurze Geburtsphase und ein rasches Eingreifen, falls es zur Dystokie kommt. In dieser Arbeit wurden AnÃ¤sthesieprotokolle von 220 HÃ¼ndinnen retrospektiv analysiert. Das Ziel war, zu evaluieren, welche AnÃ¤sthesieverfahren zum Einsatz kamen und wie sich diese auf die NarkosestabilitÃ¤t der HÃ¼ndinnen und die VitalitÃ¤t der Welpen auswirkten. Die hÃ¤ufigsten AnÃ¤sthetika fÃ¼r PrÃ¤medikation, Einleitung, Erhaltung und intraoperative Analgesie waren Butorphanol 42,5 %, Propofol 91,8 %, Isofluran 83,2 % und Fentanyl 56,7 %. Notfall-Medikamente wurden in 14,7 % der FÃ¤lle eingesetzt. In 87,9 % der AnÃ¤sthesien kam es zu Komplikationen. Dabei machten wÃ¤hrend der Narkosezeit die Hypotension (36,9 %) und wÃ¤hrend der Entwicklungszeit die Hypertension (18,1 %) den jeweils grÃ¶Ãten Anteil aus. Einen signifikanten Einfluss auf die Hypotension hatte die Anzahl intrauteriner Welpen vor OP-Beginn (p = 0,0225). Die Hypertension trat signifikant Ã¶fter bei alten HÃ¼ndinnen auf (p = 0,0078). Eine PrÃ¤medikation mit Sufentanil fÃ¼hrte im Vergleich zu Methadon und Fentanyl seltener zu Hypertensionen (p = 0,0428; p = 0,0429). In der ASA-Klasse I war die Wahrscheinlichkeit fÃ¼r Hypertensionen verglichen mit ASA-Klasse II, III und IV signifikant hÃ¶her (p = 0,0161; p = 0,0078; p = 0,0078). Bei 33,8 % der Kaiserschnitte wurden tote Welpen entwickelt. ReanimationsmaÃnahmen waren in 37,0 % der FÃ¤lle notwendig und hatten eine Erfolgsquote von 78,8 %. Die Totgeburtenrate betrug 12,1 %, wobei die GesamtmortalitÃ¤t bei 17,4 % lag. FÃ¼r die MortalitÃ¤t der Welpen lieÃ sich kein Faktor finden, der in der Mehrzahl der FÃ¤lle ursÃ¤chlich war. Jedoch zeigte sich ein Trend (p = 0,0647), dass eine lange Entwicklungszeit die Wahrscheinlichkeit fÃ¼r tote Welpen erhÃ¶ht.</ns1:description>
    <ns1:description language="en">Anesthesia for caesarean section has special requirements as both the needs of the bitch and puppies have to be considered. On the one hand anesthesia for the bitch must be deep enough to provide a surgical tolerance, on the other hand puppies should not be depressed, so they can start breathing and moving quickly after their development. In order to keep the depression of the puppies as low as possible only short acting anesthetics should be used and the puppies developed as fast as possible. However, a high impact on puppy survival is a short birth phase and rapid intervention if dystocia occurs. In this work, anesthesia protocols of 220 dams with cesarean section were retrospectively analyzed to evaluate the applied anesthetic protocol and if there is an impact on anesthetic stability and puppy mortality. Drugs most commonly used for premedication, induction, maintenance und analgesia during surgery were Butorphanol (42,5 %), Propofol (91,8 %), Isofluran (83,2 %) and Fentanyl (56,7 %), respectively. Emergency-drugs were needed in 14,7 % of the cases. Complications occurred in 87,9 % of anesthesia cases. Hypotension (36,9 %) was the most common complication during anesthesia. The number of puppies intra uteri at start of the surgery had a significant influence on the occurrence of hypotension (p = 0,0225), whereas hypertension could significantly be associated with the age of the dam (p = 0,0078) and the use of Sufentanil as premedication. Furthermore, in ASA-Classification two, three and four the likelihood of hypertension (p = 0,0161; p = 0,0078; p = 0,0078) was significantly higher compared to ASA-Classification one. Stillborn puppies were developed in 33,8 % of surgeries. In 37,0 % of the cases resuscitation of neonates was necessary and had a success rate of 78,8 %. The overall puppy mortality rate was 17,4 % including 12,1 % stillbirths. No factor was identified that proved to be causative for most cases of mortality. However, a long development time was trending for an increase of neonatal mortality (p = 0,0647).</ns1:description>
